Declining steam pressure

My MINI is slowly losing steam pressure. The steam shuts off very slowly when I turn off the lever, still tapering off steam for a couple seconds before coming slowly to a stop. The actual pressure in the wand is a fraction of what it once was, now making it almost impossible to make decent foam. It is inconsistent in pressure delivery also. It is a couple years old, no real servicing, and probably pretty hard water. Is this a scale issue, or something else? Suggestions?

Most probably scale.

Try removing what you can from each end. Tip, wand body, etc. See if you can identify the blocked area.

If that doesn't work, I'd remove the vac breaker, fill with citric, and blow it out the wand to see if that clears it.
